The evolving role and impact of patient navigators in serious illness care: a narrative review

Abstract

In today’s increasingly complex and fragmented healthcare system, patient navigation has emerged as a strategy to improve access to palliative care and reduce health inequities among historically marginalized populations. This narrative review aims to examine the evolution of patient navigation and its implementation across care settings to identify key themes and opportunities to optimize outcomes and reduce disparities in patients with serious illnesses.
